Ovarian cancer surgery in India is used to establish or confirm stage and remove visible disease when surgery forms part of the oncologic treatment plan. For epithelial ovarian, fallopian-tube and primary peritoneal cancers, the operative goal in suitable patients is complete macroscopic cytoreduction when this can be achieved safely, because residual disease and tumour biology influence outcome. The extent of surgery must be individualized rather than assumed from the procedure name.
Planning should be led by a gynaecologic oncologist within a multidisciplinary cancer team. Evaluation commonly includes contrast CT of the chest, abdomen and pelvis, tumour markers and review of pathology when tissue is already available. MRI, PET-CT, diagnostic laparoscopy or additional investigations may be used selectively. The team assesses disease distribution, surgical fitness, nutritional status and whether primary cytoreductive surgery or neoadjuvant chemotherapy followed by interval surgery is more appropriate.
Depending on disease extent, surgery may include hysterectomy, removal of both ovaries and tubes, omentectomy and resection of involved peritoneal or other abdominal disease; selected cases require bowel, diaphragm, splenic or other procedures to achieve safe cytoreduction. Risks include bleeding, infection, thrombosis, bowel or urinary complications, lymphatic complications and need for intensive postoperative support. Final pathology, stage and molecular or genetic testing where indicated guide subsequent systemic treatment. Severe abdominal distension or pain, persistent vomiting, major bleeding, breathlessness or acute clinical deterioration requires urgent local assessment rather than delayed travel.
