Percutaneous Coronary Angioplasty Ptca: общие сведения
Percutaneous coronary angioplasty in India, usually called PCI and historically PTCA, is a catheter-based procedure used to restore blood flow through significantly narrowed or blocked coronary arteries. Balloon dilatation is commonly followed by coronary stent implantation, usually with a drug-eluting stent, when the coronary anatomy and clinical situation make PCI appropriate. It may be performed electively for selected symptomatic coronary disease or urgently during an acute myocardial infarction.
Assessment is led by an interventional cardiology team and includes ECG, blood tests and coronary angiography, with echocardiography and other cardiac testing according to the presentation. In selected intermediate lesions, physiological measurements such as FFR or iFR can help determine whether narrowing is causing clinically important flow limitation. Intravascular ultrasound (IVUS) or OCT may be used to define lesion anatomy and optimise stent placement in selected complex cases.
PCI treats a focal coronary obstruction but does not cure the underlying atherosclerosis. Antiplatelet therapy after stenting and long-term control of cholesterol, blood pressure, diabetes, smoking and other cardiovascular risks remain essential. Risks include access-site bleeding, contrast-related kidney injury, coronary dissection or perforation, allergic reaction, stent thrombosis, arrhythmia and, rarely, heart attack, stroke or emergency bypass surgery. Ongoing chest pain at rest, severe breathlessness, collapse or suspected heart attack requires immediate local emergency care rather than planned travel.
