Reduced Intensity and Myeloablative Conditioning Regimens: общие сведения
Reduced-intensity and myeloablative conditioning regimens in India are preparative treatments given before selected autologous or allogeneic haematopoietic stem-cell transplants. Conditioning commonly uses chemotherapy and, in some protocols, total-body irradiation to reduce diseased marrow, suppress the immune system and create conditions for transplanted stem cells to engraft.
A myeloablative regimen uses higher treatment intensity and causes profound marrow suppression, while reduced-intensity conditioning (RIC) uses lower intensity and relies more heavily on the graft-versus-tumour or graft-versus-disease effect. RIC may be considered for older or medically less-fit patients, but it is not automatically safer or appropriate for every disease.
Regimen selection requires a haematology and transplant assessment covering diagnosis and remission status, previous treatment, donor and HLA information, organ function, infection screening and performance status. Risks can include severe cytopenias, infection, mucositis, infertility and organ toxicity; after allogeneic transplantation, graft failure and graft-versus-host disease are additional concerns.
