Transcatheter Aortic Valve Implantation (TAVI): общие сведения
Transcatheter aortic valve implantation (TAVI) in India, also called TAVR, is a minimally invasive treatment for selected patients with severe aortic stenosis who need valve replacement. Instead of opening the chest to replace the valve surgically, a collapsible prosthetic valve is delivered through a catheter, most commonly from an artery in the groin, and expanded inside the diseased native valve.
Before treatment, patients usually undergo a multidisciplinary heart-team assessment with echocardiography and CT angiography to confirm valve severity, measure the aortic annulus and assess coronary anatomy, vascular access and calcification. The team also reviews age, surgical risk, frailty, kidney function, previous cardiac surgery, life expectancy and whether surgical aortic valve replacement would be more appropriate.
Most procedures use transfemoral access, although alternative access may be required when peripheral vessels are unsuitable. Important risks include bleeding, vascular injury, stroke, paravalvular leak, kidney injury, heart block requiring a permanent pacemaker and the possibility of urgent surgery. Long-term follow-up includes echocardiography and management of antithrombotic treatment according to the cardiology plan.
