Urinary diversion with an orthotopic neobladder in India is reconstructive surgery most commonly performed after removal of the urinary bladder, often for bladder cancer. A new urine reservoir is created from a segment of intestine and connected to the urethra so that selected patients can pass urine through the natural urinary passage rather than a permanent abdominal stoma.
Before surgery, patients need a urologic-oncology and reconstructive assessment including staging, kidney-function testing, imaging and evaluation of urethral suitability, general fitness, continence potential and the ability to manage postoperative training. Neobladder reconstruction is not suitable for every patient. Alternatives such as an ileal conduit or continent cutaneous diversion should be discussed, especially when cancer extent, kidney or liver function, bowel disease, urethral involvement or functional limitations make a neobladder less appropriate.
Recovery requires catheter care, gradual reservoir training and regular follow-up. Cancer surveillance remains necessary when the diversion follows bladder-cancer surgery.
